This model allows the provider and you to partner together in your health care decisions by removing some barriers to care in the traditional model. Direct primary care is not like traditional medicine it is a membership based model. This model allows the provider and you to partner together in your health care decisions by removing some barriers to care in the traditional model. With a membership you will have enhanced access to the provider with same or next day appointments as well as direct access to provider via email, text or phone. This is not an insurance plan and does not have hospital coverage. It should not replace you health care insurance policies. We offer a membership model that is on a fixed monthly cost with no co-pay, deductible or co-insurance. Initial visit $150 followed by a recurrent monthly fee. Discounts provided for couples and families, call for more information.
What is included in DPC?
- Basic screenings (vision, hearing, biometrics, blood pressure)
- Comprehensive physical exams and sick care (up to 5 visits a month) Discounted cost of visit after the 5th
- Primary care management of chronic conditions (diabetes, arthritis, asthma, etc.)
- Procedures including basic labs and x-ray. Other labs and diagnostic testing are offered at a discounted rate.
- Fitness and nutrition counseling
- Discounted wellness programs such as weight loss, IV hydration or hormone replacement therapies.
